| Direct cremation with container provided by funeral home
Includes the local transfer of remains to the funeral home, staff services, securing of necessary authorizations, basic local transportation to the crematory, an alternative container for cremation, and return of the cremains to the funeral home. |

Our basic service charge includes: staff available 24 hours a day to respond to initial request for service, arrangement
conference with family or responsible party, planning the funeral arrangements, preparation and filing of necessary
authorizations and permits, coordination of services with cemetery, crematory, or other parties involved in the final disposition of the deceased. FTC: This fee
for our services will be added to the total cost of the funeral arrangements you select. (This fee is
already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains). |

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select
certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to
choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial. |

Limousines for funerals (or cremations) are to go from the pick-up address to the place of service to the final resting place and either back to the place of service or to the pick-up address or may have the limousine up to three and a half hours, whichever is shorter. If the customer asks the limousine to wait, then that is a separate job entirely and will be charged the overtime rate. |
